I’ve lost my White Card.

If you have lost your White Card, you will need to locate your statement of attainment (certificate) showing the completed white card units.

If you no longer have your statement of attainment, you will need to contact the training provider that you completed your White Card course through to request a new copy. If you are not sure which organisation you completed your training with, you can look up your USI record.

If you completed your original training with Asset College, we will have your statement of attainment on our records.

Once you have your statement of attainment, you can complete our White Card Replacement Request Form.

The cost for a replacement White Card is $50. You can pick up your White Card from our North Lakes office or it will be posted out to you.

If you cannot locate your statement of attainment, you will need to complete the White Card course.

How old do I have to be to get a White Card?

You must be 13 years of age or older to attend our White Card course and have your parents permission.

You will need a parent or guardian signature on your enrolment form. You will also need to ensure that you can provide the 100 points of identification required for confirming your identity in this course.
